Sounds like my little brother. Ugh. He's almost 15 and my mother still has to brush his teeth for him, and she pays him to take showers twice a week. In my opinion, she should be disciplining him, not rewarding his bad behavior with money. My son is lazy to brush teeth, brush his hair, shower, basically personal hygiene issues. Anyone can help?
Let him be filthy.

Then just once, force him to clean up and he'll realize how nice being clean is.
First of all, explain to him the importance of good hygiene.

Second, tell him there are consequences for not doing the above stated tasks. Take away his favorite whatever.
totally sit him down depending on how old he is. tell him girls od not find hat attractive or you will take video games and tv or cellphone away or dessert until he cleans himself and starts execising proper hygene routines.
If you just let him do what he's doing now; no bathing, no brushing teach, nothing, eventually he'll figure out that he has to change because he wont find a girlfriend and his friends wont come around anymore.
When he is sleeping immediately grab him and brush his teeth, his hair, and throw him on the tub scrubbing him all over.
Spank him. Answer mine:

Tell him to go back to the kitchen and make you a sandwich.
discipline him. tell him how important it is and help him out.
leave him alone until he stinks and realizes that he will not be wanted
